He was already competing internationally by age 16, snagging multiple medals in 2011 at both the European Junior Swimming Championships and the FINA World Junior Swimming Championships, held in Belgrade, Serbia and Lima, Peru, respectively. Paltrinieri took home a gold in the 1500-meter race and a bronze in the 800-meter at the former event, and a silver in the 1500-meter freestyle and a bronze in the 800-meter at the latter. The very next year, Paltrinieri participated in the European Aquatics Championships in Debrecen, Hungary-winning another gold in the 1500-meter relay and a silver in the 800-meter silver-and the FINA World Swimming Championships, where he snagged a silver in the 1500-meter freestyle. Also in 2012, Paltrinieri made it to the Summer Olympics finals, ultimately finishing in fifth place after superior placement in earlier rounds. In 2013, Paltinieri advanced to the World Aquatics Championships, this time only coming home with a bronze for the 1500-meter. Paltrinieri enjoyed similar successes over the years to follow, going so far as to establish a European record for the 1500-meter freestyle at the Italian Swimming Championships in Riccone, Italy in 2014, and a world record for the 1500-meter freestyle at the European Short Course Swimming Championships in Netanya, Israel in 2015.
