Twenty-five years after a young boy named Na Sang-woo awoke from surgery claiming that he felt every slice of the scalpel despite being under anesthesia, a troubled surgeon finds his life suddenly falling apart. Ryu Jaw-woo (Kim Myeong-min) is a surgeon plagued by suicidal nightmares. Recently, Ryu had been performing a delicate surgery on a female patient when she suddenly died on the operating table. Overwhelmed by grief, the deceased patient's husband has been threatening to settle the score by murdering Ruy's wife Heui-jin (Kim Yu-mi). Shortly after a childhood friend shows up after having recently returned from the United States, Ryu has a heated falling out with anesthetist colleague Jang Seok-ho (Jeong Yu-seok) concerning the use of hypnosis over drugs in the operating room. When Heui-jin shows up in the emergency room suffering from severe stomach pains, Ryu if forced to operate on his wife and remove the mysterious object that could kill her if it isn't removed immediately.
